Our Sliding Fee Program

At Ocean Mist Counseling and Wellness, we believe everyone deserves access to health care, regardless of their financial situation. No one is ever refused service because of an inability to pay. To help make our services affordable, we offer a Sliding Fee Discount Program for clients who are uninsured, underinsured, or facing limited financial means.

How the Discount Works

Your discount is based entirely on two things: your household income and your family size. We use the current Federal Poverty Guidelines to determine your rate:

• 100% Discount: If your income is at or below 100% of the poverty level, your services are completely free.
• Nominal Fee: If your income is between 100% and 200% of the poverty level, you will only be charged a small, nominal fee. (Note: If you cannot afford this nominal fee, please let us know. We can waive charges in certain situations so your care is not interrupted).

Who is Eligible?

This program is open to anyone who needs it. We base eligibility strictly on financial need and never discriminate based on race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.

How to Apply in 3 Easy Steps:

1. Get an application: You can download the application right HERE on our website, ask for one by calling the office at 541-270-4717, or request one when you schedule your appointment. Our staff is always happy to help you fill it out!
2. Verify your income: Return the completed form with simple proof of income (such as your two most recent pay stubs, a prior year W-2, or a letter from your employer) and a copy of your identification card.
3. Receive your coverage: Once approved, your discount is good for 12 months. It can even be applied to any outstanding balances you've had with us over the past 6 months.
